Be there for your community when they need you most! As a Social Worker in our Adult Social Care team, you will work within a dynamic Social Work Community Team, supporting individuals of all ages. You will use strength-based approaches to promote choice, control, and self-directed support, ensuring that people and their caregivers can manage their own needs and risks.

In this role, you will:
  • Assess individual needs and develop tailored support plans.
  • Promote independence and personalisation, enabling individuals to manage their own care.
  • Collaborate with health professionals and external agencies to ensure comprehensive support.
  • Uphold human rights and social justice, respecting the dignity and diversity of those you serve.
  • Engage in continuous professional development and reflective practice to enhance your skills.

To succeed in this role, you must have:
  • A recognised social work qualification and registration with Social Work England.
  • Experience in adult safeguarding and partnership working.
  • A full driver's licence and access to a car for work-related visits.
