Job summaryAs an Senior Clinical Nurse Specialist you will use yourexpert palliative care skills and highly experienced clinical judgement tocontribute to the excellent evidence-based specialist treatment and care thecharity provides to patients with complex life-limiting illnesses. You will beresponsible for assessing and diagnosing patients and planning and implementingtheir care to ensure optimum outcomes.You will also assist the Community Cluster Team Leader with linemanagement responsibilities and the daily clinical leadership of junior colleagueswithin the community team. The roleinvolves travel within the community setting, covering predominantly Mid Devonarea, and participating in the seven-day service provision to include on-call.

2. JOB PURPOSE

Act as an independent and supplementary prescriber,participating in Non-Medical Prescribing supervision to provide an enhanceddelivery of care.
Demonstrate, teach and role model Advanced CommunicationSkills and professionalism in your interactions with patients, families, carersand all other professionals in order to promote and maintain Hospiscare values.
Manage, assess and review, in conjunction with theteam leader, daily clinical leadership of junior staff within the communityteam, deputising for the team leader in their absence, to ensureseamless delivery of clinical care.
Design and deliver education and learning for allthose involved in palliative/ end of life care, to act as a resource to widerprofessional and care teams both internally and externally.
Suggest innovative ideas to improve the serviceprovision and take the service forward in line with Hospiscare, Devon Clinical CommissioningGroup and other Government initiatives to formulate strategies for positivechange.
Assist the Team Leader with line managementresponsibilities to promote good working relationships within the team.
Attend, guide and contribute regularly to Gold StandardsFramework (GSF)/Palliative Care and other multi-disciplinary team meetings, inorder to provide specialist palliative care knowledge and advice to healthcareprofessionals and relevant others.
Assist and contribute to Hospiscares clinicalgovernance and audit process in order to support continuing quality improvementand learning.
Assess and identify complex bereavement andcommunicate effectively with the Supportive Care team to enable a seamlesshandover into Hospiscares Bereavement Care Services.
