Job summary

CricklewoodGeneral Practice are looking for anexperienced Practice Nurse to join our team.

The role will be 3 days a week, Monday, Tuesday and Wednesday, 09:00 to 17:00.

The salary is £21 to £25 depending on experience.

Main duties of the job

Our Practice Nurse will beresponsible for public health and screening programmes, including infectionprevention and control, chronic disease management, immunisations, healthpromotion and prevention, well man/woman clinics and cervical screening.

They will embed population healthmanagement approaches within general practice and target health inequalities.

This role is integral to thegeneral practice team, as well as supporting the wider practice nurse networkand multi-disciplinary teams.

We are looking for a candidate withprevious experience working in a GP Practice.

Clinical Responsibilities

Act as theInfection Prevention and Control (IPC) lead for the practice, ensuring the IPCPolicy is implemented and maintained

Consult withpatients and their families to undertake assessments and deliver care based onpatient need

Managelong-term conditions, contributing to quality improvement and assuranceinitiatives

Develop andembed an effective care management programme for frail and housebound patients

Support public health and screening programmes,including immunisations, vaccinations, and cancer screening

Perform Cervical screening

Provide opportunistic health promotion adviceand lead on public health campaigns (e.g. flu clinics)

Provide travel advice services and vaccinations

Perform venepuncture and electrocardiography

Leg ulcer management, dopplers and compressionbandaging.

Undertake wound care/ management/dressingtechniques.

Carry out ear examination and syringing,following appropriate training and assessment.

Administer adult and paediatric injectionsunder an individualised prescription or Patient Group Direction, ensuring safestorage, rotation and disposal of vaccines and drugs.

Provide routine nursing care in accordance withclinical guidelines

Chaperonepatients where necessary and support the team in dealing with clinicalemergencies

Liaise withexternal services and agencies to ensure appropriate support for vulnerablepatients

Maintainchronic disease registers and implement an effective call/recall system

Maintainaccurate clinical records using SNOMED CT codes and contribute to QOF targets

ProfessionalResponsibilities

Recognisethe boundaries of practice and refer patients to appropriate professionals asneeded

Teach andadvise patients and their families on self-management of their conditions

Participatein clinical audits, research, and the development and updating of practiceprotocols

Work withinnational and local protocols and contribute to clinical governance

Adhere tosafeguarding duties for adults and children in line with current legislation

Participatein the management of patient complaints and learning from incidents andnear-miss events

Undertakeall mandatory training and attend a formal appraisal at least annually

Support thetraining and development of the nursing team, including HCA and GPA roles

Develop,review, and implement nursing protocols in conjunction with the practicemanager

Participatein and support the coordination of the nursing team rota
