Job summaryWorking alongside Social Workers and Social Care assessors you will be utilising a strength-based approach to promote independence and well-being. You will work in line with the Care Act to promote service users choice and control in their daily lives while respecting the key elements of the act to ensure service users can contribute to society and continue to have meaningful, autonomous and self-determined lives
Main duties of the jobActively encouragecustomer participation in the delivery, monitoring and evaluation of theservice
Initiate research,recommend best practice and implement changes in policy within a specialistarea of service
Support in thedevelopment and operation of team workload management systems to ensure thatresources address priority cases and respond to changing needs andcircumstances
Undertake complex assessments and careplanning
Carry out full functional assessments of peopleof all ages and with a wide range of disabilities. Assessments should also takeinto account the needs of carers and a carers assessment should be completedas appropriate.
Adviseon appropriate care packages and refer on to other services as necessary
Recommendand arrange for the provision of specialist equipment, ensure correctinstallation, instruct client and/or carer in safe use and monitor theeffectiveness of the provision as appropriate
Advise on and facilitate major adaptations,including major adaptations to properties through consultation with serviceusers, Housing department, Environmental Health services, builders and otherprofessionals as appropriate
