An exciting opportunity has arisen for a Band 5 Neonatal Homecare Nurse to work within Neonatal Home care Service

This model of Neonatal Community Care is provided across Leicester and Northamptonshire region, including Hospitals of Leicester, Kettering and Northampton Hospitals . Posts currently available for the Leicester, Leicestershire & Rutlands area

The post holder will be an excellent team worker and will be responsible for ensuring highest standards of clinical care and evidence based practice for infants within the scope of neonatal outreach, providing specialist on-going care for the neonate and advice, support and education to the family in the home environment.

We are delighted to offer a 7 day service and have well established services including Home phototherapy for jaundiced babies, Home Oxygen and Home naso gastric tube feeding

The Neonatal Home careTeam provides a service covering 7 days a week, from 08:00-1700Monday to Sunday

We are seeking a highly motivated Neonatal Nurse to join our forward thinking family focused neonatal team. The successful candidate will demonstrate drive and motivation to support expansion of such an exciting service. To be successful in this role you will need to be registered nurse/midwife , ideally a post specialist neonatal course QIS experience, current Health visitor or Paediatric community care would be considered

Main duties of the job

The job holder will assist Neonatal Unit, transitional care based staff with early and appropriate discharge planning in accordance with the criteria for outreach care.

To provide a seamless timely discharge from hospital to the home environment. Providing and delivery of parent education programmes including Basic life support.

Work without direct supervision, including the assessment of care needs, plan, organise and implement care at home ensuring continuous provision of integrated care.

To provide on-going holistic care within the community setting for vulnerable neonates and their family , ensuring equitable care across diverse communities

Provide specialist on-going care for the neonate and education, advice and support to the family in the home environment.

Monitor nutrition, feeding skills and growth, ensuring onward referral when indicated .Monitor and plot growth and identify when intervention is required to improve growth .

Recognise , prioritise and respond appropriately to urgent and emergency situations

Take responsibility for the care needs of individual allocated patients, referring to the Band 6/7 when necessary for advice .

Provide evidence of work diary , identifying planned visits in accordance with lone worker policy

Monitor and inform of relevant changes of care plan required to meet the health needs of our babies following assessment and discussion of any changes to their condition by appropriate escalation

Work as one team , providing cross site cover as required
