Detailed job description and main responsibilities Develop and facilitate others in promoting an evidence based learning culture and practice, supporting new starters and students.
Recognise , prioritise and respond appropriately to urgent and emergency situations
Take responsibility for the care needs of individual allocated patients, referring to the Band 6/7 when necessary for advice .
Provide evidence of work diary , identifying planned visits in accordance with lone worker policy
Monitor and inform of relevant changes of care plan required to meet the health needs of our babies following assessment and discussion of any changes to their condition by appropriate escalation
Work as one team , providing cross site cover as required
• The job holder will take a proactive role in the education and training of the multidisciplinary team(s), providing on-going supervision and support where neonates require on- going care needs within neonatal outreach.
• To work effectively and efficiently as an Outreach team member as well as independently in the planning, assessment, implementation and evaluation of neonatal community care.
• Report safeguarding concerns, follow safe guarding advice, initiate action/referrals where appropriate.
• To be confident and competent within the Outreach setting as a lone worker in accordance with trust policy
• Ensure that fully accurate, complete and contemporaneous nursing records are written and maintained for each patient
Person specificationTraining & Qualifications Essential criteria - Registered Nurse Child , Adult branch/ Midwife
Desirable criteria - QIS in Neonatal Care
- Post registration training in community care of the neonate
- Neonatal Foundations course
- Teaching and assessing qualification
Experience Essential criteria - Minimum of 2 years Neonatal (or relevant post registration
- Experience eg Health Visiting or Community Paediatrics
Desirable criteria Communication and relationship skills Essential criteria - Good standard of spoken and written English
- Good listening and communication skills
- Diplomacy

The Trust will pay for the check initially and the money will then be deducted from your salary over a three month period commencing on your first month's payment.
The current price of a check is £38 for an enhanced and £18 for a standard check.
