Job description

Job summary

The post holder will work as a member of the Overnight Nursing Service providing direct and indirect patient care as directed by a trained member of the team in order to support patients and their families within their own home environment. The post holder will undertake clerical duties as directed including electronic documentation and working as an effective member of the Multidisciplinary team.

Due to the lone working policy the post holder will never work alone and will always be with a band 6 specialist nurse.

Main duties of the job

  • To provide basic nursing, therapy and social care to people and carers, carrying out delegated clinical, therapeutic and personal care tasks
  • To promote the physical, mental and emotional health and wellbeing to individuals and client groups.
  • To be an effective member of the multi-disciplinary team.
  • Communicate effectively with patients and carers, recognising their needs for alternative methods of communication.
  • To carry out care/other duties under direct supervision of a registered professional.
  • To support the band 6 nurse with tasks in patients homes.

  • Maintain excellent written patient records, and demonstrate the principles of patient confidentiality in accordance with Caldicott guidelines.
  • Recognise and respond to challenging behaviour.

To maintain close liaison with colleagues and to communicate appropriately with other departments/services as required. Ensuring effective feedback regarding patients conditions to appropriate health care professionals

If successful this vacancy will require an enhanced DBS check at a cost of £54.90. This cost will be deducted from your first Trust salary

In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £16
