Job summary

An exciting opportunity has arisen for a compassionate and motivated Band 5 Community Nurse to join our Community Adult Twilight & Night Service based in Brent.

Our dedicated team provides high-quality, responsive nursing care to patients within their own homes, helping them to remain independent while preventing unnecessary hospital admissions. We are committed to delivering safe, evidence-based and patient-centred care, ensuring every patient receives the highest standard of support.

This is an excellent opportunity for a registered nurse who enjoys working autonomously within the community while being part of a supportive multidisciplinary team.

We are recruiting nurses to work the following shifts:
  • Twilight Shift: 4:00pm - 12:00 midnight
  • Night Shift: 12:00 midnight - 8:30am

Main duties of the job

The Community Nurse will provide a mixture of general and specific care to a group of patients under the guidance and supervision of a registered nurse or allied health professional. The Community Nurse will work in patient/carers own homes as well as visiting patients in residential homes.

The post holder will be expected to become proficient and competent in their area of practice by undertaking competency assessments specific to the area of clinical practice. The Community Nurse will promote optimum independence and individualised client centred care to a delegated caseload.

The post holder will also support the Community Nursing Team to deliver against key performance indicators and maintain quality of the service. The service is provided seven days per week 4pm-Midnight and Midnight-8.30am covering the five localities across Brent
