We are excited to announce an opportunity for a Community Care Officer to join our Mental Health Discharge Hub on a fixed-term contract until March 2027. This role is perfect for those eager to enhance their skills and gain valuable experience in mental health services. You will play a vital role in assessing individuals eligible for Section 117 aftercare, working closely with Integrated Care Board (ICB) colleagues and partner agencies.

As a Community Care Officer, your responsibilities will include:
  • Assessing and Reviewing: Conducting assessments and reviews of care and support for individuals.
  • Coordinating Care: Arranging and monitoring commissioned services to ensure quality support.
  • Collaborative Working: Engaging with multidisciplinary teams to promote effective discharge planning.
  • Information Sharing: Gathering, interpreting, and presenting information clearly, both verbally and in writing.
  • Participating in Meetings: Actively contributing to multidisciplinary meetings and partnership discussions.

To thrive in this role, you will need:
  • A relevant qualification in health or social care (e.g. NVQ Level 3 or equivalent) or evidence of relevant skills and experience.
  • Knowledge of community care legislation
  • Proficiency in using social care recording systems and IT applications.
