Job summaryWe are seeking an experienced and highly motivatedAdvanced Nurse Practitioner (ANP)to join our multidisciplinary team atFarnham Park Health Group. The role will focus on providing high-qualitypatient care by supporting clinical staff in managing acute and chronicconditions, undertaking home visits, and contributing to Quality and OutcomesFramework (QOF) achievement work. To support the management team in the reviewing of clinicalpolicy and procedure, including support to more senior members of the clinicalteam. In order to work at these level MSC requirements foradvanced practice must be met.
Main duties of the job - Be the first point of contact for patients presenting with undifferentiated, undiagnosed problems. Making use of skills in history taking, physical examination, problem-solving and clinical decision-making to establish a diagnosis and management plan
- Refer patients to alternative care setting where appropriate.
- Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports.
- Provide patient education on managing health conditions and preventative care.
- Ensure clinical correspondence is appropriately managed
