Job summaryAn exciting opportunity for experienced neonatal nurses to develop their leadership skills and experience in a Neonatal Transitional Care and Community Outreach Band 6 Junior Sister/Charge Nurse post.

We are looking for dynamic individuals who can adapt their neonatal skill set to the postnatal ward setting and the community. The Transitional Care team is an exciting service which involves the team providing a model of transitional care to babies, who are being nursed alongside their mothers within the maternity service. This supports reducing newborn separation and the maintenance of the family unit. It involves working across both neonatal and maternity unit boundaries and with the wider MDT.The Saint Mary's MCS Neonatal Outreach service involves visiting families within their homes, supporting clinics, meeting with families before discharge to facilitate a high quality effective and efficient transition from hospital to home.

Professionallyattendbabiesbeing discharged fromtheneonatalunitandbeingcaredforathome,orthosebabieswithaspecialcaredependencyonthe post-natal wards.
Promotehealtheducationactivitiestoparentsandfamilies.
Liaisewiththemultidisciplinaryandmulti-agencyteamstoensurethatbabies andtheirfamiliesreceiveahighqualityservice,coordinating andparticipatingin case discussions as required.
Leadbyexampleandactasapositiverolemodelforallstaff,takingresponsibilityforpersonal and professionaldevelopment.
Maintainpersonalstandardsofconductandbehaviour,whichareconsistentwithTrustandNursingandMidwiferyCouncil(NMC)standardsandrequirements.
Ensurealldocumentationisaccurate,contemporaneousandcomplieswithNMCstandards.
Support theMNORT Clinical LeadandMatronforContinuingCareinrelationtothetransformationagendawithintheclinicalsetting,includingeffectiveleadership,thesetting,monitoringandmaintenanceofprofessionalstandards,collaborative working,the management of complaintsand leadingthenursingcontributiontoquality and safetyatadepartment/unitlevel.
