Therapy Assistant Practitioner - Promoting Independence Team (24 Hours per Week)
Help People Live Independently and Achieve What Matters Most to Them We are seeking an enthusiastic, motivated, and compassionate Therapy Assistant Practitioner to join our Promoting Independence Team on a 24-hour per week basis.
This is an exciting opportunity to become part of a forward-thinking service committed to supporting adults to maximise their independence, build on their strengths, and achieve the outcomes that matter most to them. You will work alongside Occupational Therapists and Social Care professionals to deliver person-centred interventions that promote wellbeing, prevent escalation of needs, and reduce reliance on long-term services wherever possible.
As a Therapy Assistant Practitioner, you will play a key role in helping people regain confidence, develop daily living skills, and remain safe and independent within their own homes and communities.
Therapy Assistant Practitioner Promoting Independence Team As a Therapy Assistant Practitioner, you will support the delivery of high-quality Occupational Therapy interventions for adults with physical, cognitive, sensory, and mental health needs. Working under the guidance of Occupational Therapists, you will complete assessments, identify and implement intervention plans, support equipment provision, and help individuals achieve meaningful goals that improve their quality of life and wellbeing.

The Therapy Assistant Practitioner will: - Carry out strengths-based assessments to identify individuals' needs, goals, risks, strengths, and priorities, ensuring timely and proportionate interventions that promote independence and prevent, reduce, or delay the need for ongoing care and support.
- Support and empower individuals to maximise their quality of life, achieve personal outcomes, and develop the skills and confidence needed to remain independent within their own homes and communities.
- Work collaboratively with individuals and carers to identify what matters most to them, recognising their strengths while addressing any physical, environmental, social, or cognitive barriers to independence.
- Liaise effectively with health, social care, voluntary, and third-sector partners to coordinate person-centred interventions, contribute to joint assessments, and support integrated care planning.
- Undertake functional assessments and apply strengths-based practice principles to identify practical solutions that enable people to achieve their goals and maintain independence.
- Assess, recommend, prescribe, and review equipment, minor and major adaptations, and Technology Enabled Care (TEC) solutions to address social care needs and improve functional outcomes.
- Provide specialist advice and guidance to colleagues and partner agencies regarding equipment, adaptations, housing solutions, and reablement approaches that promote independence.
- Maintain accurate and timely electronic records, reports, and case documentation to support effective service delivery, performance monitoring, and service development.
- Contribute positively to the development and continuous improvement of the Promoting Independence Team.
About You - Experience working within a health, social care, rehabilitation, reablement, or community-based setting.
- Understanding of the principles of promoting independence and strengths-based practice.
- Experience supporting adults with physical disabilities, long-term conditions, frailty, cognitive impairment, or mental health needs.
- Excellent communication and interpersonal skills.
- Ability to build effective relationships with adults, carers, families, and professionals.
- Good organisational skills and the ability to manage a varied workload.
- Ability to work both independently and as part of a multidisciplinary team.
- Commitment to providing person-centred and outcome-focused support.
- Good IT skills and experience maintaining accurate records.
Desirable Requirements - NVQ Level 3 (or equivalent) in Health and Social Care, Occupational Therapy Support, Rehabilitation, or a related subject.
- 5 GCSE's or equivalent
- Experience working alongside Occupational Therapists.
- Knowledge of community equipment and minor adaptations.
- Experience delivering reablement or rehabilitation programmes.
- Understanding of Adult Social Care legislation and safeguarding procedures.
