Full & Part Time / 37 & 18.5 Hours Per Week / Permanent & Fixed Term for 12 Months due to backfilling secondment

Are you an experienced support worker with enthusiasm and creativity in your approach? If so, this is a great opportunity to support children and their families to identify and arrange suitable short break activities, by working in partnership with other agencies and services.

The Disabled Children's Service supports children and young people aged 0-19 years who have a range of physical and learning disabilities and challenging behaviours to access short break services in a variety of settings including services within their own home.

You'll be required to work across the city to visit service users and attend meetings, working flexibly including evenings, weekends, and school holidays due to the nature of the role.

What you'll be doing

As a support worker, you'll undertake short break assessments to identify needs and formulate person centred support packages. This will include identifying and arranging suitable short break activities and enabling children to develop their skills and make friends, providing the parents and carer's a valuable break from their caring roles

What you'll need

You'll have previous experience of working with children and young people with disabilities and complex needs. With strong understanding of inclusion and equality issues, and the development needs of children, you'll effectively create, arrange, and support a range of suitable activities.

Using your excellent verbal and written communication skills, you'll facilitate group and individual work, complete short break enquiries and assessments, and review short break packages. With a respectful and tactful manner, you'll motivate change and work positively in a team.

With proficient IT skills, you'll be able to use software such as Liquid Logic and Early help Module systems to record information for your caseload.

This role is subject to an Enhanced DBS check.

National Care Minimum Standards apply under The Equality Act 2010, Schedule 9 Part 1 so Applicants must be aged 21 and over.
