What you will do:

• Provide support to individuals experiencing substance misuse issues, helping them on their journey to recovery.
• Develop and implement personalised recovery plans in collaboration with clients and other healthcare professionals.
• Conduct assessments and deliver one-to-one sessions to support clients in overcoming challenges.
• Engage with community resources and partner organisations to strengthen and enhance client recovery plans.
• Maintain accurate records to ensure the continuity and quality of care.
