Join our Mental Health Discharge Team! This dynamic role focuses on supporting individuals in acute psychiatric wards. You will play a vital part in assessment, discharge planning, and enabling timely, person-centred transitions from hospital to home. Your expertise will help individuals regain their independence and improve their mental health outcomes.
As a Social Worker, your key responsibilities will include:- Assessment and Planning: Conduct thorough assessments and create tailored discharge plans.
- Reviewing S117 cases
- Collaboration: Work closely with individuals, families, carers, and partner organisations to understand their needs and preferences.
- Commissioning Care: Identify and arrange necessary care and support for safe discharges.
- PromotingIndependence: Encourage individuals to live independently through reablement and community-based support.
To thrive in this role, you will need:- A recognised Social Work qualification.
- Registration (or eligibility to register) with Social Work England.
- Familiarity with community care services and relevant legislation in adult social care including the Mental Health Act.
