INTERNAL VACANCY

If you are an internal candidate interested in applying for this job as a secondment opportunity, please discuss with your current line manager before submitting your application.

As part of a wider transformation programme, we are redesigning our early support service to better manage demand and ensure people receive the right advice, support and intervention at the earliest opportunity. Our ambition is to create a more responsive, strengths-based approach that maximises independence, prevents escalation of need, and delivers the best possible experience for those accessing adult social care.

The redesigned service will bring together a multi-disciplinary team, including Social Workers, Occupational Therapists, Telecare Specialists and Local Area Coordinators, to review contacts collaboratively and resolve issues wherever possible at the first point of contact.

About the Role

This is a unique opportunity to shape the development of the service and bring the valuable skills and expertise of the Local Area Coordination model into the heart of Adult Social Care.

The successful candidate will:
  • Play a key role in embedding prevention and early intervention within Front Door operations.
  • Expand and strengthen the team's knowledge of community assets, networks and resources.
  • Develop stronger links and visibility within local communities.
  • Support a proactive approach to building community resilience and reducing reliance on formal services.
  • Contribute to the day-to-day operational management and decision-making processes within the team.
  • Provide line management and support to the team.
  • Work closely with colleagues across the multi-disciplinary team to promote strengths-based practice and innovative solutions.

What We're Looking For

We are seeking an experienced and motivated Senior LAC who:
  • Has a strong understanding of community development, asset-based approaches and prevention.
  • Is passionate about enabling independence and supporting people to achieve their goals within their communities.
  • Has excellent leadership, communication and partnership-building skills.
  • Can confidently contribute to operational decision-making in a fast-paced environment.
  • Is able to support and develop staff through effective line management.
  • Enjoys working collaboratively across professional boundaries to achieve positive outcomes.

Why Apply?

This secondment offers a fantastic opportunity to influence the future direction of Adult Social Care's Front Door service, contribute to an innovative and evolving model of practice, and further develop your leadership experience within a dynamic multi-disciplinary team.

If you are passionate about prevention, community-based solutions and improving the experience of people accessing support, we would love to hear from you.
