Person specificationEducation Essential criteria - GCSE English language level 4-9 (or equivalent)
Desirable criteria Experience Essential criteria - Previous health care experience experience
- Experience of using MS Office, Word, Excel, and outlook
- Experience of working with Multi-Disciplinary Teams and/or attending meetings
Desirable criteria - Demonstrate an understanding of discharge planning
- Experience of working in the NHS, use of NHS systems and understanding of medical terminology
Knowledge Essential criteria - Demonstrate understanding of GDPR and working with sensitive and confidential information
Desirable criteria - Red2 Green/ Criteria to Reside / SAFER initiatives
- Local Discharge Planning processes
Skills Essential criteria - A self-motivated, innovative, and a pro-active problem solver
- Able to meet tight deadlines, prioritise workload, and work under pressure
- Flexible in approach to meet the challenges of a trust wide post
- Demonstrate good standards of written and verbal communication
- Ability to communicate at all levels
- Ability to work independently or as part of a team
