Person specificationEducation/Qualification Essential criteria - Registered Nurse on the NMC register or other health and social care registered professional
- Educated to degree level
- Evidence of continuous professional development
Experience Essential criteria - Knowledge of National Framework for NHS Continuing healthcare and Nursing Funded Care and associated legislation and guidance
- Relevant experience supporting people with complex health and care needs, including experience applicable to older people's services
- Substantial relevant post-registration experience, with evidence of working autonomously with complex clinical needs
- Proven clinical Leadership Skills
- Experience of establishing & maintaining good working relationships
- Experience of working across multi- agency services
- Experience of holistic assessment
Skills & Knowledge Essential criteria - Ability to communicate, influence and negotiate constructively, including when perspectives differ or change is required.
- Ability to assess patients with complex needs
- Ability to lead case conferences/MDT meetings
- Ability to work autonomously within professional boundaries and seek appropriate support or escalation when required
- Ability to manage competing priorities
- Able to manage own workload
- Able to communicate clearly and effectively both written & verbally
- Able to deal with challenges and appeals with the support of the Clinical Lead
- Awareness of Deprivation of Liberty Safeguards -Experience of completing Mental Capacity Act
