Job summary

We are seeking a motivated, flexible, and skilled practitioner to join a dynamic and supportive team providing community rehabilitation to adults experiencing complex psychosis. The service promotes a positive team culture where learning, reflection, and professional development are actively encouraged, alongside access to a range of training opportunities.

This role is suitable for an experienced Nurse, Occupational Therapist, or Social Worker with experience working with individuals with enduring mental health conditions.

The post holder will deliver high-quality, comprehensive mental health services with the primary aim of improving the psychological health and wellbeing of individuals with complex psychosis within a designated Devon locality.

Working within a Recovery and Rehabilitation Service, the role focuses on delivering recovery-oriented, evidence-based interventions and providing support to service users and their families.

Main duties of the job

• Work across the Torbay and South West Devon locality
• Manage a defined caseload, delivering specialist assessment, treatment, and recovery-focused care for individuals with complex psychosis and their carers
• Lead on designing, implementing, and reviewing personalised care
• Deliver evidence-based interventions and utilise specialist knowledge, including substance misuse, to support service users and the wider team
• Contribute to assessment, care planning, risk management, and review processes within the multidisciplinary team
• Work collaboratively with carers, partner agencies, and professionals across Torbay and South West Devon
• Coordinate and facilitate therapeutic activities and structured rehabilitation pathways
• Promote engagement, independence, and social inclusion through a strengths-based, person-centred approach
• Maintain accurate and timely clinical records in line with Trust standards
• Provide clinical support and supervision to team members and contribute to service development and evaluation
• Hold a reduced caseload to enable delivery of intensive rehabilitation interventions
• Demonstrate effective organisation and time management in a dynamic working environment

Essential

• Extensive post registration experience working at band 5 or 6 in mental health.
• Experience working with individuals with complex psychosis or enduring mental health conditions
• Experience of delivering recovery-focused, person-centred care
• Experience managing a caseload, including assessment, care planning, and risk management
• Experience working within a multidisciplinary team
• Experience maintaining accurate and timely clinical records
• Experience engaging service users who may be reluctant or hard to reach
• Ability to work independently and manage competing demands in a pressured environment

Desirable

• Experience working within inpatient mental health or rehabilitation settings
• Experience of recovery coordination or care programme approach (CPA) processes
• Experience delivering evidence-based psychosocial interventions
• Experience or knowledge of working with co-occurring substance misuse
• Experience contributing to service development, quality improvement, or evaluation
• Experience providing supervision, mentorship, or informal leadership within a team
• Experience facilitating therapeutic groups or structured activities

Qualifications

• A professional mental health qualification eg RMN, Social Work Degree or equivalent, BSc/Diploma in OT, SROT, Chartered Clinical Psychologist, on the appropriate professional body register and hold current registration with no restrictions Evidence of relevant continuing professional development

Desirable

• Post graduate qualification relevant to specialism
• Clinical Supervision qualification
• Training to level 3 in Safeguarding children and young people: roles and competencies for Health Care staff Evidence of leadership/management training within a core service
