What you will do

• Caseload Management: Manage a caseload of service users, providing tailored recovery support.
• Assessment & Planning: Conduct comprehensive assessments, including risk evaluations and recovery planning.
• Care Delivery: Deliver one-to-one sessions and facilitate group-based recovery programmes.
• Harm Reduction: Support service users to reduce harm, improve wellbeing, and access treatment services.
• Safeguarding: Promote safeguarding practices and respond appropriately to concerns involving vulnerable individuals.
• Multi-Agency Working: Coordinate support with healthcare providers, housing, and external agencies.
• Record Keeping: Maintain accurate, confidential, and timely case records.
• Empowerment: Encourage service users to access education, employment, and community opportunities.
• Team Contribution: Participate in team meetings, supervision, and ongoing professional development.
