Job Introduction

Reporting to: Frailty Team Manager Hours:
37.5 hours per week
Salary: £29,542 Location: Memorial Hospital, Shooters Hill, Woolwich Closing Date: Mid-day 23rd September 2026

Do you have experience in health and social care or third sector information and advice? Have you helped to deliver a service to people with complex needs? Are you comfortable working with minimal supervision?

We are seeking applications from individuals with experience of delivering services in the third sector, health or social care for people with complex needs. You will need to demonstrate a compassion and empathy for people in a professional manner, have excellent communication, organisational and IT skills and experience of building relationships across the sectors.
