Job summary

An exciting opportunity has arisen for an enthusiastic and motivated Registered Nursing Associate to join the Specialist Diabetes Team at Wye Valley NHS Trust.

This is a fixed-term position for two years and offers a varied Band 4 role working across inpatient wards, outpatient clinics, including specialist antenatal diabetes services, and within patients' own homes, supporting the delivery of high-quality, person-centred diabetes care.

Working under the direction and supervision of the Diabetes Specialist Nurses and wider multidisciplinary team, you will support the delivery of specialist diabetes services for adults with diabetes. A key aspect of the role is supporting people to use diabetes technology, including continuous glucose monitoring (CGM) and flash glucose monitoring systems, enabling them to develop the knowledge and confidence to self-manage their condition.

The role also includes supporting the delivery of education for patients, carers and healthcare professionals, contributing to structured education programmes, assisting with service audits and quality improvement projects, and promoting best practice in diabetes care across the Trust.

This is an excellent opportunity for a Registered Nursing Associate with an interest in diabetes who is keen to develop specialist knowledge and skills within a supportive and forward-thinking team.

Main duties of the job

Working within the Nursing Associate scope of practice and under the direction of the Diabetes Specialist Nurses, the post holder will:
  • Your main duties will include:
    • Supporting patients with the initiation and ongoing use of diabetes technology, including continuous glucose monitoring (CGM) and flash glucose monitoring systems.
    • Providing one-to-one and group education to patients, carers and healthcare professionals to promote effective diabetes self-management.
    • Supporting specialist diabetes clinics and inpatient services, working alongside the multidisciplinary team.
    • Downloading and reviewing glucose monitoring data, escalating concerns appropriately and supporting the Diabetes Specialist Nurses with patient management.
    • Assisting with community visits to support patients in their own homes where appropriate.
    • Contributing to service audits, quality improvement projects and the ongoing development of the diabetes service.
    • Maintaining accurate clinical records and working within the Nursing Associate scope of practice to deliver safe, compassionate and effective care.
