Job summary

An exciting opportunity has become available for a Band 6 Nurse Practitioner to join the newly commissioned Unscheduled Care Coordination Hub (UCCH) at Wirral University Teaching Hospital.

This exciting role offers the chance to lead and influence the delivery of a new, innovative model of urgent care coordination that is transforming the way patients access community-based services.

The UCCH has been developed to centralise and standardise the management of Urgent Community Response referrals, including Category 3, 4 and 5 referrals received from Northwest Ambulance Service (NWAS). You will provide expert clinical oversight, manage complex referrals, utilise Directory of Services, and ensure patients are connected to the most appropriate care pathway quickly and effectively.

If you are passionate about integrated care, service innovation and improving patient outcomes, this role offers a unique opportunity to contribute to a service that is shaping the future of urgent and community services.
