Job summaryWelcome to FCMS!
We are looking for band 6 practice nurses andnon-medical prescribers to join the team on a bank basis, covering all theservices within Inclusion Health.We arelooking for individuals with a passion and enthusiasm to work with some of themost vulnerable and complex people within our community. The right candidateswill be approachable and display excellent communication skills, compassion,tolerance, and a non-judgemental attitude.
Main duties of the jobDay to Day Duties to include, but notexhaustive:
To provide proactive reviews and support for patients with CVD and / orCOPD in line with clinical competence and local pathways
To initiate prescribed medication and amend medication when required.
Undertake holistic health assessments, identifying physical, mental andsocial needs
Deliver health prevention and health promotion interventions includinglifestyle advice (alcohol, smoking, diet and physical activity), weightmanagement and wellbeing support, Opportunistic screening and vaccinationadvice
Identify unmet needs and escalating health concerns, taking appropriateactions
Deliver care within the mobile clinical unit adapting consultations to acommunity-based environment
Support people who present to the mobile unit by providing assessment andadvise where appropriate, signposting and referring to the most suitableservice when out the scope of practice and offering reassurance, healthinformation and ongoing support
Promote self-management and empower patients to engage with healthservices
Accurately record all activity, assessments and outcomes on EMIS usingappropriate clinical coding and templates
Maintain confidentiality and data protection standards
Maintain Current NMC registration adhering to the NMC code of conduct
Work autonomously withing scope of practice, escalating concernsappropriately
Participate in supervision, appraisal and continuing professionaldevelopment

Job description Job responsibilitiesCompass Medical Practice is a GP practicefor those who are difficult to manage in mainstream practice. Patients areeither consulted via telephone/ video link or face to face. When a face-to-faceappointment is required, a risk assessment is undertaken to assess what supportthe clinician requires. Consultations are supported with the use of patientliaison officers and consultations are carried out in our mobile clinic van orin an agreed prebooked venue.
Compass Medical Practice also provides medical coverto temporary patients residing at
Thornton House. These are shortterm patients that are residing at Thornton house for either rehab, rest orrecouperation. There is a weekly ward round and multidisciplinary team meeting.Compass Medical practice also offers medical support throughout the week,outside of the ward round, during opening hours.
Our
homeless health service is nurse led and isoperates on a Monday, Tuesday, Thursday and Friday from the Salvation Army incentral Blackpool. The nursing team take a multidisciplinary approach, oftenworking with wider services to meet the needs of a complex patient cohort who arefacing multiple disadvantage.
The
Targeted demand management (TDM) service supports people across Blackpool to address clinical andnon-clinical factors increasing their risk of an unplanned hospitalisation.
Key aims of TDM are to provide clinical and non-clinical interventionsthat meet the needs of individuals identified in Blackpool with the following respiratoryor cardiovascular long-term conditions. The TDM service, through reducingindividual risk, will play a role in relieving pressure on acute services,through preventing health crises.
