Are you a motivated,GPHC-registered Pharmacy Technician with an Accuracy Checking qualification looking to make a real difference to patient care?We are seeking an enthusiastic and motivated Pharmacy Technician to join our Pharmacy Homecare Team.

This is an exciting opportunity to join a collaborative and forward-thinking pharmacy service. You will support the delivery and development of our Homecare Medicines and Outpatient Parenteral Antimicrobial Therapy (OPAT) services, helping to ensure the safe, accurate and timelysupplyofmedicinestopatientsthroughhomecareproviders.Thehomecareservicecurrentlysupportsmorethan2,000patientsacrossCheshireandcontinuestoexpand,providingopportunitiestodevelop specialistknowledgeinagrowingareaofpharmacypractice.

Main duties of the job

As a key member of the Pharmacy Homecare Team, you will support the delivery and ongoing development of the Trust's Homecare Medicines and OPAT services.

This includes coordinating homecare prescriptions, patient registrations, renewals, orders, and records, while ensuring compliance with relevant legislation, policies, and professional standards.

You will act as a point of contact for patients, homecare providers, clinical teams and other stakeholders, responding to enquiries and helping to resolve service, delivery, and medication related issues. You will contribute to the monitoring of homecare provider performance, investigate incidents and complaints, and support service improvement initiatives.

The role also includes ordering, dispensing and accuracy checking medicines, supporting medicines reconciliation activities, and counselling patients. You will maintain accurate records, contribute to audit and governance activities, and support service improvement initiatives.
