Job summary

The Gastroenterology department is currently basedat WRH for in-patient work and cross site(WRH/Kidderminster/Redditch/Evesham/Malvern) for out patient work. Mostgastroenterology services are provided including diagnostic endoscopy (OGD,colonoscopy, flexible sigmoidoscopy, push enteroscopy), therapeutic endoscopy(including EMR in the upper and lower GI tract, dilatation, stenting, PEGplacement, varices banding etc), ERCP, EUS and video capsule endoscopy. The IBDservice is currently run by 4 specialist nurses and includes a helpline andbiologics service (with over 1000 patients). There are 2 surgeons with aspecial interest in IBD and a monthly IBD MDT. We also have a liver CNS whoruns specialist clinics, the fibroscan service and is trained to do paracentesis.The ward is currently supported by a Tier 1 Physicians associate and 2 Tier 2Physicians associates to help run the service.We have weekly team meetings, monthly governance meetings, upper and lower GIcancer MDT and GI pathology MDT meetings. There are weekly GIM grand rounds.

Main duties of the job

The post will involve regular endoscopy and clinicsessions at Worcester, Kidderminster and Redditch together with some in-reachto in-patients in Redditch requiring gastroenterology review. It is expectedthat the post holder will be competent in OGD and flexible sigmoidoscopy orcolonoscopy (this will normally mean holding JAG accreditation unless theapplicant was fully trained prior to the availability of JAG accreditation).Therapeutic OGD to treat upper GI bleeding is also expected.In addition to these duties, the appointee will provide cover for consultantcolleagues who are on leave

.On call commitments will be 1 in 9for endoscopy (predominantly GI bleeding) on the Worcestershire Royal Hospitalsite and the Alexandra Hospital Redditch site. This is covered in 1 week longblocks where 2 consultants do either 2 or 5 on calls during the week. There isalso a commitment to provide a gastroenterology ward round on a Saturdaymorning when on call and in-reach into ED/MAU on a Sunday morning. Thesefrequencies will fall in the future with the appointment of other posts.

There is also a second on consultant who covers Advice and Guidance, in-patientreferrals (supported by our registrars), the upper GI MDT, a rapid accessclinic. There is an occasionalrequirement to review a small numbers of patients (typically < 5) who aremedical outliers at WRH. Where required,this additional work is remunerated at trust agreed hourly rates of payment.

A suitablyexperienced post-holder will be expected to provide a clinical service in therelevant specialty to patients of the Worcestershire Acute Hospitals NHS Trust and to General Practitioners.

The post-holder willbe expected to provide a service to patients of the Worcestershire AcuteHospitals NHS Trust, General Practitioners and to attend multi-disciplinarymeetings. The successful candidate will be expected to work withinmulti-disciplinary teams in providing high quality clinical care. It isanticipated work will be undertaken on three sites and closer to the patients home. The Trust is seeking toextend services to GP surgeries/health centres etc and job plans may be revisedin due course.

The post-holder willprovide support to the A&E department and acute medicine to ensure timely appropriate care and patient flowthrough the hospital.

The post-holder willprovide consultation and advisory service to clinical colleagues in the Trustand Primary Care.

The post-holder willattend cross county and cross sitemeetings.

The post-holder willcollaborate with colleagues to deliver ahigh quality, timely service and to work towards meeting the Trusts and departmental objectives.

The post-holderwill, if relevant, participate in the on call rota to ensure that bothemergency and urgent services areprovided out of normal hours, to beshared equally with Consultant colleagues in post.

The Trust wouldexpect a consultant whilst undertaking on-call duties to attend the hospitalsite to review patients whenclinically necessary. The consultant on call must be prepared to attendwillingly whenever requested to do so by a member of their medical and nursingteam.

The post-holder,whilst undertaking weekend on call duties, will attend the hospital to undertake patient reviews.
