Our Ollerton Day Opportunities team is looking for a passionate Hub Facilitator to join us in making a positive impact on the lives of adults with various needs.

As a Hub Facilitator, you will play a pivotal role in enhancing the lives of adults attending our day services. Your focus will be on providing high-quality, person-centred care that promotes independence, wellbeing, and inclusion. You will plan and deliver engaging activities that help individuals achieve their personal goals and connect with their local communities.

In this rewarding role, you will:
  • Provide Personalised Care: Deliver high-quality physical and emotional support while respecting the dignity and individual needs of each person.
  • Plan Activities: Create and implement engaging activities that foster independence and social connections.
  • Maintain Care Records: Keep accurate records of care plans and risk assessments, ensuring compliance with health and safety regulations.
  • Administer Medication: Safely manage medication administration in line with our policies.
  • Support Team Culture: Collaborate with colleagues, families, and professionals to ensure tailored support for each individual.
  • Respond to Emergencies: Handle crisis situations effectively and escalate issues as necessary.

To succeed in this role, you should have:
  • A Social Care qualification or equivalent, ideally working towards or holding a Level 3 Diploma in Adult Care.
  • Knowledge of relevant adult care legislation and standards.
  • Evidence of continuous professional development and mandatory training.
