Are you an acute children's nurse who's looking to branch out into community nursing? Or a Children's Community nurse looking for more acute focussed nursing care at home?

The Royal London Children's Hospital is looking to recruit or second a band 6 children's nurse to cover maternity leave to support delivery of the innovative and exciting hospital @ home service commissioned by Tower Hamlets CCG. The team visit acutely unwell children in the community to offer treatment preventing them from attending hospital and/or staying in longer than needed. The H@H team forms part of the Pan London H@H forum with its additional support and links that helps to provide a cohesive and high quality service to the children of Tower Hamlets.

The successful candidate will have excellent team working and decision making skills and put their patients first. We are looking for experienced and highly skilled children's nurses who can work autonomously to care for acutely unwell children at home. Main duties of the job
• To facilitate efficient and effective patient flow from attendance to hospital to referral home, helping to ensure that we meet the 4 hour wait target in the emergency department.
• To perform highly skilled acute nursing care and assessment for children at home
• To work closely with the ward managers, paediatricians, matrons and flow team on a daily basis to expedite referrals home
• To act as an excellent role model and leader to all staff
• To participate in the development of new pathways of care
• To support the evaluation of the service regularly and adapt as necessary

Qualifications

Essential criteria

• CN or RN (Child)
• Relevant First level degree or masters in appropriate subject or working towards

Desirable criteria

• Recognised teaching/ mentorship qualification
• Previous audit experience

Experience

Essential criteria

• Evidence of Work based learning and ability to meet competencies
• Significant post registration experience within an acute or community nursing setting.
• Experience of facilitating clinical decision making and the application of professional judgement in clinical situations
• Awareness of professional responsibilities

Desirable criteria

• Completion the Advanced assessment of the presenting child Module at MSc level
• Community Qualification
• Independent prescribing or commitment to work towards

Knowledge

Essential criteria

• Knowledge of current issues relating to patient safety
• Knowledge of current legislation affecting health
• Good knowledge of anatomy and physiology and the management of an acutely unwell child

Skills

Essential criteria

• Excellent clinical nursing skills pertinent to the role
• Ability to effectively prioritise and organise own workload
• Assertively manages situations which may be complex and involve conflict or crisis.
• Experience of the application, of professional judgement in a range of clinical and non-clinical situations.
• Excellent interpersonal skills, is engaged and able to negotiate and mediate
• Communicates with clarity and direction, both written and verbally
• Analyses and identifies trends in clinical information, identifies issues and proposes solutions to problems

Desirable criteria

• IT skills, use of data and spread sheets, production of clear reports, action plans and presentations
• Venepuncture and Cannulation skills
