Qualifications

Essential criteria

• NMC/HCPC registered with current valid registration

Desirable criteria

• Experience of working in Community Services
• Physical Clinical Assessment Skills Course or willingness to undertake the course.
• Experience of assessing and providing quality care to people with Long Term Conditions

Experience

Essential criteria

• Substantial B5 Experience
• Experience of assessing and providing quality care to people with long term conditions
• Extensive range of clinical skills and experience
• Experience of working in a multidisciplinary team

Desirable criteria

• Experience of supervising staff and completing appraisals

Skills & Knowledge

Essential criteria

• Ability to communicate effectively verbally and in writing
• Ability to make decisions and prioritise actions
• Ability to manage own workload and plan the workload of others, to work autonomously
• Basic IT skills including use of Microsoft Office package and email
• Clinical knowledge across a range of long term and enduring conditions
• Knowledge of evidence-based practice and able to implement in practice

Desirable criteria

• Knowledge of clinical governance processes, assessing and managing risk
• Knowledge of current NHS organisational structures and policy initiatives

Key Attributes

Essential criteria

• Able to carry out the duties of the post with or without adaptions.
• Able to work flexible shifts
• Able to work across the four neighbourhoods which make up South West Hertfordshire.
• Requirement to hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless the candidate has a disability as defined by the Equality Act 2010).
