We are a specialist 0 - 19 yrs Level 3 - targeted service, offering intervention to families who need a holistic, intensive approach, including working as part of a multi-agency team, to help prevent the need for social care intervention.

We encourage parents and carers to lead in creating their own sustainable family plan, with their child/children at the core.

We want to be there for families as early as possible so that we can help them to overcome their challenges and stay together.

We support families with difficulties in the following areas (based on level 3 of the Level of need tool).

An Introduction to the role and key responsibilities:The role involves providing intensive support to children, young people and their families who have multiple and complex needs, supporting them with appropriate skilled interventions, both practically and emotionally. This will involve working with families in the home and community in close partnership with other agencies. This may be in response to a crisis or for some families as part of a support package. We aim to strengthen the family's network, to increase the support from their own families and communities around them.

As a Family Practitioner you will provide a vital role to the families referred to the service; assessing needs, developing action plans, setting and reviewing targets, providing interventions and coordinating the input from other service providers. As well as keeping accurate, up-to-date electronic records of your work, you will attend and lead the Team around the Family meetings.

We use the Family Outcome Star Plus and Graded Care Profile 2 to support families in creating their plan and to help them find the right path to move forward. Both tools are strengths-based and focus on the journey of change. We also deliver evidence-based parenting programmes to groups of parents and their teenage children as part of our service.

What sorts of skills do you need?Previous experience of working with families who have multiple and complex needs and a working knowledge of the safeguarding of children and vulnerable adults is desired.

You should be empathetic and understanding, but at the same time, persistent, tenacious and assertive. An ability to listen and offer practical solutions, an honest approach, and to support and challenge where needed, to enable people to make positive changes in their lives.

You will need a flexible approach to working hours, as there may be a need to work some evenings and weekends.
