Job overviewWe are seeking a motivated and compassionate District Nurse to join our community team. This is a rewarding opportunity to make a real difference by delivering high-quality care to patients in their own homes.
Essential Criteria:- District Nursing qualification (DN SPQ)
Key Responsibilities:- Assess patients in their homes, developing, implementing, and evaluating individualised care plans
- Manage a caseload with the support of the Clinical Lead
- Provide leadership and line management to a team of Registered Nurses and Healthcare Assistants
- Work flexibly across teams to meet changing service demands and ensure continuity of care
Our core service operates
8:00am - 6:00pm, 7 days per week, and flexibility is essential to support the needs of the service.

Main duties of the jobWhat you will be doing - A District Nursing qualification (DN SPQ) is an essential criteria.
- Assessing new patients for nursing care in their own homes, creating and evaluating care plans.
- Managing a caseload with support of the Clinical Lead
- Line Management and leadership of a team of Registered Nurses & HCAs.
- Our core service hours are 08:00 to 18:00, 7 days per week.

Person specificationQualifications Essential criteria - Community Specialist Practitioner Qualification - District Nursing
- RN part 1 on NMC register
- Nurse prescriber / V100
- NMC required teaching & assessing module, or commitment to undertake
Experience Essential criteria - Teaching in a clinical setting
- Experience of supervising a team
- Experience of caseload management
- Experience of multidisciplinary working
Desirable criteria - Experience of managing change and developing teams
