Job summary

The purpose of this role is to deliver a comprehensive, skilled and evidence-based structured education service to meet the needs of the population of adults with Type 2 Diabetes in multiple settings.

In addition, the post holder will support registered nursing and dietetic staff in the management and execution of clinical practice. They will also carry out duties delegated and supervised by a registered nurse to provide a high standard of care and to promote a good image of the service.

Main duties of the job

  • To provide a comprehensive community diabetes education service for patients aged 18 and above who are referred to the community Diabetes Specialist Nursing Service.
  • Deliver education programmes to health and social care staff that support people living with diabetes either in their own homes or in residential and nursing home settings.
  • As a skilled educator, the post holder will assist in delivering the framework for structured diabetes education programmes to increase diabetes knowledge and skill levels.
  • The post holder will work autonomously, prioritising workload, be accountable for their own actions and demonstrate initiative and an ability to work without direct supervision.
  • The post holder will be an excellent communicator, responsible for delivering high quality structured diabetes education either in a group setting or individually.

Essential

  • NVQ level 3 in health-related subject or equivalent
  • Diploma in healthcare related subject
  • The post holder must be willing to undertake the relevant training for the post

Experience
