Job summary

An exciting opportunity has arisen to join our established and highly respected Critical Care Outreach Team. The team delivers a 24/7 service across Stoke Mandeville and Wycombe General Hospitals at Buckinghamshire Healthcare Trust.

We are looking for a highly motivated, innovative and critical care practitioner to deliver patient-focused care to acutely ill, deteriorating and critically ill patients across the trust. The successful candidate will need to display excellent communication, interpersonal and leadership skills, be able to respond to rapidly changing, high pressure situations and priorities. You will be able to work autonomously and as part of a team and have good critical thinking and decision making skills. You will be instrumental in the early recognition of deterioration and deliver an effective response, liaising with the patient's own team. You will be able to facilitate smooth transitions of care for patients stepping down from Intensive and High Dependency Care to the general wards and be involved in the provision of a range of education for the multidisciplinary team.

Main duties of the job

The post holder will be an experienced practitioner providing early intervention to acutely unwell adults utilising appropriate assessment and observation skills to develop treatment plans. Under supervision of the senior Critical Care Outreach (CCO) team, they will assist in promoting the development of expanded practice throughout the Trust whilst ensuring the role of the CCO service evolves to meet changing demands.

The following objectives for Critical Care Outreach Services (DoH, 2000) are key components of the role:

o Avert admissions by identifying patients who are deteriorating and either helping to prevent admission or ensuring admission to a critical care bed happens in a timely manner

o Enable discharges by supporting the continuing recovery of discharged critical care patients and their significant others.

o Share critical care skills with staff in wards, ensuring enhancement of training opportunities and skills practice and to use information gathered from the ward and community to improve critical care services for patients and relatives.
