The Opportnity

ECC Adlt Social Care is making a transformational shift from a focs on long-term care spport, to a model with for key principles: prevention, early intervention, enablement, and safegarding. Enabling people, to access information and tools, to live independently and allowing a positive response to growing service demand in a sstainable way, within the financial constraints of ECC.

Working with Social Workers and Occpational Therapists, delivering bespoke intervention work, in accordance with the care plan, to vlnerable adlts. The role may be reqired to manage a caseload, incldingassessment of risk throgh direct work, within a qality assrance process. A focs of the role will be to ensre vlnerable people are safegarded in line with ECC's stattory obligations.

With relevant experience of working with vlnerable adlts or edcated to Level 3 (RQF). Yo will be passionate abot engaging with and providing spport in the commnity, working with partners as reqired.

Accontabilities

Assess social care needs and provide spport to vlnerable adlts, to enable them to live healthily and independently; maintaining high standards of professional practice which contribte to continos improvement across the service and delivering the for key principles.

Contribte to the development of care and spport plans that gain commitment to soltions to meet each individal's needs.

Works collaboratively with others, inclding placement team, to spport the individal to commission care and spport from appropriate providers; inclding developing their own employees throgh Direct Payments.

Accontable for working within the stattory legal framework, policy and gidance and determining when the threshold for stattory intervention has been reached in In line with legislative and organisational standards within the Care Act 15 (e.g., MCA / DoLs / CHC etc.).

Responsible for highlighting isses and implementing plans to safegard a vlnerable adlt.

Bilding strong, effective relationships with clients, families and carers providing practical advice, spport and, where necessary, acting as an advocate. Enabling vlnerable adlts to live healthily and as independently as possible.

Work collaboratively across the service, with internal and external professional partner agencies to deliver joint objectives and ensre improved otcomes for vlnerable adlts.

Provides information and advice in relation to involvement/accessing of third party agencies relevant to the needs of the client (e.g., benefits agencies, immigration, and hosing).

Attends meetings, reviews, case conferences, and cort hearings to provide information, insight and evidence; providing reports where appropriate.

Specific individal and shared targets and objectives are defined annally within the performance management framework.

The Experience Yo Will Bring

Experience within a social care setting, an NVQ/BTEC Level 3 or eqivalent is desirable.

Evidence of contining professional development and knowledge in relevant professional area.

Experience of bilding strong, effective relationships with adlts, their families, and carers in times of challenge and crisis.

Experience of ndertaking direct, activity based work with adlts.

Knowledge of legislative and policy gidance applicable within Social Care settings.

Proven track record of bilding working relationships with partners and external agencies.

A certificate in First Aid and/or knowledge of Sign Langage is desirable in this role.
