Qualifications

Essential criteria

• Higher Professional Diploma in Health and Wellbeing (NVQ/QCF - Level 3)
• A recognised equivalent course
• Willingness to undertake further training as appropriate

Desirable criteria

• GCSE grade A-C or equivalent in English and Maths
• ECDL certificate
• Registration with the national regulatory body (when it becomes available).
• PSI qualification

Experience

Essential criteria

• Experience of working in a health or social care setting
• Experience of working as part of a team
• Experience with the use of Microsoft Office and a variety of databases.

Desirable criteria

• Experience of working as part of a team
• Experience with the NHS patient information database (SystmOne, Rio, CIS)

Skills & Knowledge

Essential criteria

• Knowledge of NHS An awareness of risk management.
• Demonstrate an acceptable level of numeracy and literacy.
• Knowledge of multiagency services available and processes for referral
• Knowledge of the effects of the ageing process and needs of vulnerable adults in relation to their health and well-being
• Self-management skills-managing time and workload
• Ability to analyse, interpret and record data in a clear, and succinct manner
• Good analytical and problem solving skills
• To be able to promote the work of the team across the trust
• Effective interpersonal, influencing and negotiating skills
• Adaptable and flexible
• Ability to cope with uncertainty and change

Essential criteria

• Flexible about work location - the post holder will be required to travel to other CLCH sites in London
• Appropriate level of physical fitness to perform the job (as outlined in the job description) with or without limitation
