Person specificationQualifications Essential criteria - Higher Professional Diploma in Health and Wellbeing (NVQ/QCF - Level 3)
- A recognised equivalent course
- Willingness to undertake further training as appropriate
Desirable criteria - GCSE grade A-C or equivalent in English and Maths
- ECDL certificate
- Registration with the national regulatory body (when it becomes available).
- PSI qualification
Experience Essential criteria - Experience of working in a health or social care setting
- Experience of working as part of a team
- Experience with the use of Microsoft Office and a variety of databases.
Desirable criteria - Experience of working as part of a team
- Experience with the NHS patient information database (SystmOne, Rio, CIS)
Skills & Knowledge Essential criteria - Knowledge of NHS An awareness of risk management.
- Demonstrate an acceptable level of numeracy and literacy.
- Knowledge of multiagency services available and processes for referral
- Knowledge of the effects of the ageing process and needs of vulnerable adults in relation to their health and well-being
- Self-management skills-managing time and workload
- Ability to analyse, interpret and record data in a clear, and succinct manner
- Good analytical and problem solving skills
- To be able to promote the work of the team across the trust
- Effective interpersonal, influencing and negotiating skills
- Adaptable and flexible
- Ability to cope with uncertainty and change
