Job summary

We have a great opportunity for a full-time band 6 nurse to work Dementia & Older People's Mental Health Team based in Horsham, West Sussex. The service provides an Intensive Support service for those experiencing crisis operating a 9-5pm 7 days a week including Bank Holidays and a Lead Practitioner role where the post holder manages their own caseload working 9-5 Mon- Friday excluding B/H's.

The successful candidate will join a friendly, highly skilled and enthusiastic multi professional team. We will take a keen interest in helping you to develop the skills important to this role and the post-holder will be encouraged to develop special interests and expertise in line with service and team objectives. You will be part of a highly skilled and supportive multi-disciplinary team. You will receive regular managerial and clinical supervision. This is a challenging but rewarding work as we receive positive feedback from our service users and carers.

The post holder will be expected to travel across the Crawley, Horsham and Chanctonbury area covered by the team, the ability to travel to remote areas is essential. You must have a car and a valid driving licence to apply for this post.

Main duties of the job

The role of this post is to actively engage those requiring an increased level of support with the aim of keeping people within their home and maintaining their wellbeing and independence as far as possible. We are looking for individuals who are dynamic, compassionate, good communicator who are able to demonstrate a holistic and person-centred approach to their work.

You will also need to be able to work cohesively with other services both within our Trust, and with other partner agencies. We follow a multi-disciplinary approach and within that context you will manage a caseload of older adults.

This post offers a Golden Hello, or a Relocation package up to a maximum of £2000. (pro rata applies and subject to terms and conditions. Please see attached Protocol document).

  • To provide specialist assessment and intervention as part of a multi-disciplinary approach, with specific responsibilities regarding the care pathway
