We are looking for enthusiastic and motivated Community Care Officers to join our Urgent and Emergency Care Team. In this role, you will support individuals transitioning from hospital to home, ensuring they receive the right care and support to live independently. You will assess needs, coordinate services, and promote personalisation, helping our community thrive.

As a Community Care Officer, your key responsibilities will include:

• Assessing and reviewing the needs of individuals and their carers.
• Coordinating solutions and interventions to meet personalised outcomes.
• Identifying opportunities for reablement and enablement services.
• Promoting independence and personalisation in care.
• Providing information, advice, and support to service users and their families.
• Completing Mental Capacity Act assessments when necessary.
• Managing your social care workload independently with oversight from your line manager.
• Collaborating with health professionals and community resources to enhance service delivery.

To succeed in this role, you will need:

• A relevant Health/Social Care qualification (NVQ Level 3 or equivalent).
• Knowledge of community care services and legislation, including the Care Act 2014 and Mental Capacity Act 2005.
• Experience in assessing needs and coordinating care services.
• Proficiency in using IT applications for record-keeping and reporting.
