We are looking for enthusiastic Community Care Officers to join our Urgent and Emergency Care (UEC) Service in the South of the County. This is an exciting opportunity to work in a dynamic team, helping individuals transition safely from hospital to home. You will play a crucial role in supporting people to live independently and achieve their personal goals.

As a Community Care Officer, your key responsibilities will include:
  • Attending Multi-Disciplinary Team (MDT) meetings to assist with discharge planning.
  • Conducting assessments and reviews in homes and care units, focusing on strengths-based approaches.
  • Supporting carers and fostering professional relationships.
  • Contributing to a duty rota, including some weekend work to meet service needs.
  • Collaborating with colleagues and managers to achieve timely outcomes.

To thrive in this role, you will need:
  • A relevant health or social care qualification (e.g., NVQ Level 3 or equivalent).
  • Knowledge of community care services and legislation related to adult social care.
  • Proficiency in using IT applications for case management and reporting.
