As a Community Care Officer, your responsibilities will include:

• Identifying Goals: Work with clients to discover their personal goals and aspirations.
• Coordinating Care: Organise a range of outcomes to meet individual needs.
• Promoting Independence: Encourage clients to take control of their own care and support.
• Collaborating with Professionals: Work alongside other professionals to provide comprehensive care.
• Managing Caseloads: Effectively manage your caseload to ensure timely and quality support.

To excel in this role, you will need:

• A relevant qualification in social care or a related field.
• Experience in adult social care or a similar environment.
• Knowledge of strength-based approaches and person-centred care.
• 2 years of adult care experience
