The post holder will work as a member of the Mental Health Home Treatment Team for people who are living with dementia or age related mental illness and who are experiencing a mental health crisis.
This post holder will contribute to the Occupational Therapy component in the Home Treatment Service and also generic responsibilities within the team. The focus of the role will be that of individual assessment, intervention and evaluation of treatment, delivered within the framework of NICE guidelines, evidence based best practice and to maximise service user's ability to cope with their mental health crisis and regain functional abilities. A key role will be to strengthen the discharge coordination function of the acute care pathway, supporting the inpatient and community teams to plan discharges in a safe and timely manner.
The service works closely with psychiatric liaison services in the Boroughs and inpatient services, making this an ideal opportunity for an enthusiastic Occupational Therapist who wants to work in a new team and influence how care is delivered for this client group. The applicant will be expected to complete Gatekeeping assessments in the acute Hospital environment with patients who present with high levels of clinical risk and acute mental health difficulties. This will require sound clinical reasoning skills and good problem solving skills.
To do the job effectively you will need to:
Demonstrate good communication skills
Have sound experience of working as an OT with older people with dementia or acute mental illness in a community, or an inpatient setting
Have an understanding of the complex range of health and social care services that contribute to effective community care for older people
Be able to work flexibly, creatively and as part of a team
