Job summaryAn exciting opportunity for a Band 6 nurse to join our existing OPAT team for a fixed term period.
We are looking for a dynamic, driven individual who can assess individual care needsfor patientsrequiring on-going intravenous (IV) antibiotics within their own homes post discharge from the acute trust.

Main duties of the jobUnder the supervision of the Lead CNS the newly appointment member will
Complete competency assessment duties of patients from the emergency portals, out-patient clinics and the wards. With the support provided it will enable the patient to access the correct pathway to avoid admission and reduce length of stay.
Collate and format requested data.
Support admission portals and inpatient wards with knowledge and understanding of both intravenous and oral antibiotics as per advise from the MDT and advances in Antimicrobial Stewardship.
(AMS)Assess patients progress, review weekly/twice weekly blood monitoring and liaise with the MDT or referring consultant for additional support and advice.

- 3 years post registration experience
- Degree Level
Desirable - Health Assessment in Clinical Practice, Independent and Supplementary Nurse/Midwife Prescribing Non-Medical Prescribing
Personal Attributes Essential - oEffective interpersonal and intrapersonal skills including high degree of self-awareness and self- regulation
Desirable - Evidence of leadership education/training
Experience Essential - Ability to work autonomously and be accountable for own actions
- Excellent written and communication skills
- Acts to support and enable effective teamwork
