We're looking for a motivated, organised, and compassionate CareCoordinator to join our collective Integrated Community Teams with partner organisations - a dynamic group of clinicians, practitioners, and community partners all dedicated to delivering outstanding, joined-up care.
Main duties of the jobAs a Care Coordinator, you'll be at the heart of our multidisciplinary team (MDT), ensuring everyone stays connected and informed. Your role will include:
- Coordinating MDT meetings and streamlining communication
- Supporting proactive, personalised care planning for patients with complex needs
- Working closely with GPs, nurses, therapists, social care, and voluntary sector partners
- Helping the team deliver seamless, patient-centred care across our neighbourhood
- Keeping systems updated and making sure the right information flows to the right people at the right time

Person specificationQualifications Essential criteria - Qualifications Good level of general education including A levels and GCSE or equivalents including functional skills level 2 Evidence of continued professional development to NVQ level 4 in relevant subject or equivalent level of knowledge and experience
- Two-day coaching or motivational interviewing course and the elearning below or complete (or working towards) a PCI accredited course for care coordinators. Personalised care and support planning Shared decision making To have completed or working towards a portfolio of CPD underpinned by the competencies outlined in Competency framework for care co-ordinators
Desirable criteria - ECDL or alternative IT qualification Healthy weight coach training Person centred approaches
Experience Essential criteria - Experience of computerised record systems and data storage / retrieval / analysis More than 3 years' experience of working in health, social care or information and advice, in direct contact with people, families and carers in a paid or voluntary capacity
