Job summary

We are looking for a dynamic and compassionate Bank Band 7 Urgent Clinical Practitioner to join our Integrated Front Door Team, an innovative service at the forefront of transforming urgent care delivery, improving patient transitions and reducing system pressures. This role offers a flexible opportunity to support safe, timely and effective patient care at the front door, focusing on preventing unnecessary hospital admissions, reducing length of stay and promoting proactive, preventative care through early identification and intervention. Working as part of a fully integrated multidisciplinary team including consultants, GPs, pharmacists, nurses, therapists and healthcare support workers, you will provide advanced clinical assessment, decision-making and leadership, supporting admission avoidance, early supported discharge and improved patient flow across acute, community and partner services. We are seeking an experienced professional who is able to work autonomously within a fast-paced environment and shares our commitment to delivering high-quality, person-centred care, contributing to a responsive and resilient service.
Main duties of the job

You will undertake advanced clinical assessment, diagnosis and decision-making for patients presenting at the front door, managing complex and undifferentiated presentations and supporting safe, timely patient care. You will play a key role in admission avoidance by identifying and directing patients to appropriate community pathways, working collaboratively with multidisciplinary teams across acute, community, local authority and partner services to support patient flow. The role includes providing clinical leadership, managing a caseload autonomously, contributing to MDT discussions and discharge planning, and ensuring a person-centred approach that promotes early supported discharge and care closer to home.

  • Current registration with NMC or HCPC without restrictions on practice.
  • Evidence of health assessment/ physical examination qualification.
  • Evidence of post-registration, working towards Master's Level (L7) qualification appropriate to the post or equivalent experience.
  • Demonstrable clinical supervision training or management experience or qualification.
  • Demonstrates continuing professional development within area of urgent and emergency care.
Experience

Essential

  • Significant relevant post registration experience.
  • Demonstrable recent experience in acute, emergency, urgent community response, or admission avoidance services.
  • Experience of supervision or management of junior staff.
  • Experience of compiling protocols and clinical guidelines
Knowledge and Skills

  • Evidence of advanced clinical practice skills including advanced physical assessment, cannulation, phlebotomy, and interpretation of investigation results.
