What you will do:

• Provide occupational therapy services as a Care Coordinator, supporting service users with their individual needs.
• Develop and implement personalised care plans to promote independence and wellbeing.
• Work collaboratively with multidisciplinary teams to ensure effective and coordinated care.
• Use EPJS to maintain accurate and up-to-date patient records.
• Engage with service users and their families, providing guidance, support, and education.
• Travel within the Croydon area to deliver services effectively, requiring a valid driver's licence.

Qualifications/Requirements:

• Previous experience working as an Occupational Therapist within a relevant setting.
• HCPC registration as an Occupational Therapist.
• Experience using EPJS is desirable.
• Ability to travel within the Croydon area with a valid driver's licence.
