Making a Difference
Community Facilitator Volunteer will support with the facilitation and development of the Support at Home Community Hubs. You will support patients their families and carers, creating a welcoming a friendly environment.
Create a welcoming and friendly environment
Keep records of group attendees
Prepare and serve teas, coffees and drinks
Facilitate group activities
Provide support for the patient in coping with the effects of their illness and how it impacts on their day to day life.
Ensure any issues or concerns are communicated to the Service Lead

Someone with good listening and communication skills.
Able to understand the effects of a patient's illness and how it impacts on them and their families/carers.
Able to work in a positive and professional manner
Someone who can support individuals on an individual and group basis
