We have a rewarding opportunity for you to join our Transforming Care team, supporting our brilliant customers to live a fulfilling life in their own home within the local community. Our team support our customers who have complex support needs, including learning disabilities, mental health issues, autism and communication differences, alongside ongoing physical health conditions. They may also have a forensic background and distressing behaviours.
The gentleman you will be supporting has a fun, bubbly and energetic personality and loves the outdoors, taking part in lots of adventurous activities from hiking, biking to rock climbing. He also loves a wide range of sports including running, tennis, football, darts, snooker, swimming and lots more! In fact, he'll have a go at most! His other interests include music, singing, acting and impersonating; so if you love outdoor activities too and are combination of the X Factor, Britain's Got Talent and a wannabee Olympian, well you're just perfect for our role! Of course, we're only kidding and what he really wants, is a Team Leader that will take the time to get to know him, who is caring, understanding, a good listener, positive and encouraging. Don't worry there won't be an audition and you'll find no Simon Cowell on our interview panel!
With the help of our clinical team including our Positive Behavioural Support Specialist, you'll help him to participate in his favourite things as well as trying new things through positive risk taking. You will need to be able to provide consistent support when he is finding things a bit difficult as well as find creative ways to help him participate in daily living activities aimed at increasing his independence.
You'll initially support the transition of our customer from Cardiff to his permanent home in Swindon, so for the first few months, there will be some regular travel and working from Cardiff which will include some overnight stays when required (this of course will be shared out across the team). Once he has moved into his lovely new home, you'll manage our small team of frontline Positive Behavioural Support Workers in providing support that is person-centred, understands someone's past life and experiences and builds on their strengths. Together, you'll be supporting him to enjoy the best quality of life and live as independently as possible.
As our team leader (known internally by us as Positive Behavioural Support Team Leader) with responsibility for safeguarding, you'll use your experience and brilliant skills in effectively motivating and directing colleagues. You'll ensure our exciting new LIFE support practice model (Living Independently and Feeling Enabled) is delivered in a safe, effective, caring, responsive and well-led way. We'll tell you all about it when we meet you! You'll also oversee complex support, working with our customer when he displaying any distressing behaviours and any requirement for personal care.
About our team
You'll report to Deb Duggan our Registered Manager who has worked for Home Group for 5 years. Deb has loads of experience in working with customers with both mental health and learning disabilities . Deb has a clinical background in nursing and was an accredited psychotherapist in a previous life before joining Home Group. Outside of work you'll find her supporting her local football team, spending quality time with her family and loves cooking for friends.
Where you'll work
You'll work primarily with our one gentleman customer initially from Cardiff up to a few days a week and then in his own home in Swindon, as well as supporting him in the community. But there may be opportunities to work with our other customers in Swindon! You can use public transport to support him and we'll pay your expenses to cover this!
Our customer needs support 24/7 to meet his needs, so you'll take your turn with your team working on a rota which could include working days, evenings, weekends, bank holidays and waking nights . You may also be required to do sleep ins or be on call in which an enhancement will be paid. As we're asking you to be flexible for our customer, we'll also give you the time back that you need to spend time doing the things that matter to you! Deb will help you develop the rotas that provide the support needed in the most flexible way.
You'll have an excellent working knowledge and experience in CQC Regulated services and have an awareness of the national Transforming Care agenda. It's important to us that you have worked in a Senior Support Worker role before, with experience of supporting people who may show distressing behaviours. Holding a Level 3 Diploma in Adult Health & Social Care or equivalent, you'll have a great understanding and passion for promoting independence and social inclusion through person centred support. You'll provide personal care in a strengths-based way to facilitate greater independence, as well as use creative solutions, and assistive technology where available to support independence. To be brilliant you will have previous experience in supporting people using positive behavioural support.
To be awesome in this job, your key strengths will lie in people management and motivation, bringing colleagues together to share information, best practice, fostering creativity and overall ensuring our customer has a brilliant team! You won't be left to it! Deb and our clinical team will be regular visitors to provide you with all the support you need!
When you join, you'll need to have a valid Enhanced Check under the Disclosure and Barring Service. The great news is that we will pay for that to be done if you don't have a transferrable one.

Don't worry, we won't drop you in the deep end, we'll nurture your talent, help you understand how things work here and you'll have a fantastic induction with ongoing learning and support! You'll be trained to support our customer's mental and physical wellbeing and to take positive risks to enable his successful journey to having his own home and being part of his community. If required (and only as a last resort), you may need to use physical intervention and we will provide you with training and support to do this safely.
We want you to get the right balance and to be and feel always at your best to continue to deliver brilliant outcomes for our customer. So your wellbeing and taking time back when you need it too is our top priority!
