An opportunity has arisen for a permanent Band 5  Clinical Nurse Practitioner based in South Camden Rehabilitation and Recovery Team, Kentish Town.  South Camden Rehabilitation and Recovery Team is one of the largest teams in the trust. We are a busy multidisciplinary team comprised of nurses, occupational therapist, social Workers, benefits advisor and employment specialist and psychiatrist's. Community Recovery & Rehabilitation Teams provide multi-disciplinary treatment to service users with psychosis. working to implement a recovery approach as the guiding principle of their way of working with service users. We work closely with individuals, their families, the local authority, London borough of Camden to provide support and treatment .The successful post holder will act as care co-ordinator for a defined group of clients, you will be the named worker for an assigned caseload. You will work with clients in their own home and in clinic and/or hospital settings. We also have an intake team that manages new referrals and supports clients open to the South Camden Rehabilitation and Recovery Team. All staff participate in a rota to work in the intake team.  The service utilises a team approach and has a multidisciplinary team structure. Other development activities include peer support, regular supervision and participation in training programme.
