Islington's Services for Ageing & Mental Health Community Mental Health team has a vacancy for a Clinical Nurse Practitioner to work with older people who have dementia and/or severe and enduring mental illness and with their carers. This post is an ideal opportunity for any nurse who is passionate about working with older people and who wishes to further their career and gain valuable experience working in a community setting.
The successful candidate will work as part of our integrated, multi-disciplinary service, supporting service users' recovery, helping to maintaining independence and providing health and social care. The post holder will proactively manage a caseload of service users who have complex needs, coordinating personalized health and of social care packages, providing specialist nursing assessments and interventions.
The post holder will have the ability to work autonomously and as part of a multi-disciplinary team. You will receive training and professional supervision relevant to the role and support to develop specialist skills working with older people who have mental health needs.
A female staff member is required as the team need to ensure that female patients have appropriate choice of who administers depot medication and other intimate care tasks
