About this vacancy
We are looking to appoint a highly motivated, innovated and enthusiastic senior pharmacist to join the BSUH pharmacy team in the role of Acute Frailty\/Elderly Care lead pharmacist.
The post holder will:
Provide an expert level of clinical pharmacy services to acute frailty and elderly care across all sites over seven days at BSUH
Lead specialist clinical pharmacy practice for acute frailty and elderly care and be responsible for the delivery, development, evaluation and co-ordination of clinical pharmacy services in line with the objectives of the division and the BSUH pharmacy strategy.
Take responsible for all aspects of medicines management to optimise patient's care involving the use of medication.
Participate in the education and continual professional development of undergraduates of both medicine and pharmacy and healthcare staff in all aspects of elderly care
Empower patients to make shared decisions regarding their medicines and health, optimize treatment regimens and reduce pill burden in line with deprescribing and medicines optimisation strategies.
Communicate to resolve ambiguities relating to care of the elderly patient's medicines across the primary \/ secondary care interface. Actively works to improve the 'transfer of care' of elderly patients across the interface with the view to reducing medication related readmissions
At Brighton & Sussex University Hospitals (BSUH) pharmacy we believe that for pharmacy services, average is not good enough. We are looking to appoint talented pharmacists who are self-directed, productive team-players able to manage the workload and pressure of this key NHS role. You will be responsible for the delivery of expert patient care to our acute frailty patients. You will also provide support to the divisional lead pharmacist for the development, evaluation and co-ordination of clinical pharmacy services in line with the objectives of the BSUH pharmacy and acute\/specialty medicine division strategy. You will contribute to all aspects of clinical governance, including clinical risk and audit. As a teaching hospital, education and training is an integral component of all roles encompassing practice supervision of undergraduates, pre-registration pharmacists and the line management and educational supervision of band 6 and 7 pharmacists. We encourage inter-professional learning and have been recognised for educational initiatives such as joint teaching activities with Foundation Doctors. Our most recent review by Health Education England of our Pharmacy department education infrastructure concluded that 'our trainees felt both well supported by the senior staff members and that they were very approachable'. Brighton and Sussex University Hospitals is the regional teaching hospital, providing general acute services for local patients and tertiary services for patients across Sussex and the south east of England. Working as one hospital across two main sites in Brighton and Haywards Heath, we treat over half a million patients a year.
You will be working alongside our highly experienced pharmacists who are well respected within the Trust with many recognised nationally and internationally for their expertise within their specialist areas. The Pharmacy department has established links with the University of Brighton, University of Sussex, Brighton & Sussex Medical School and University Teaching Hospital, Lusaka, Zambia as part of the Brighton-Lusaka Health Link. We have recently been awarded a grant from the Commonwealth Partnerships to champion pharmacists as antibiotic guardians in Zambia.
